As the rounds wore on and more punishments were eliminated, the host would come to offer you a deal in place of what was in the mystery box. Being electrocuted for example. Or needles - though the description of that one was not all the specific.
If you did not take his deal, you were left with the punishment that was in the original box you had chosen.
